**Dogs must be 12 months or older to receive a Cardiac OFA exam number

| Uses electrodes to record the electrical activity of the heart, evaluates heart rate and the sequence with which the heart becomes electrically active. | Passes a beam of sound into the chest and records waves, reflected from the heart, that permit seeing the heart contract, valves open and close, and blood flow. |
PLEASE NOTE: Advanced Exam OFA Forms will be available for completion at the clinic. You need to know the following for each dog to be examined:
- Your dog’s AKC registration number and Microchip number
- Your dog’s Sire and Dam’s AKC registration number
AUSCULTATION ONLY: You need to complete and print out the OFA Exam Form to bring with you to the clinic. The form can be found here: https://ofa.org/applications/. There is no capability to photocopy forms at the clinic. Vet information to complete the form will be available at the clinic.
**Pre-Registraion is NON-REFUNDABLE.**
You will be contacted by email to schedule an appointment time ON OR AFTER the date the Judging Schedule has been published. Exhibitors are contacted in the order of pre-registration, “first come, first serve.” It is important that you respond ASAP to secure an appointment time because as we work down the list of pre-registrations, appointment times fill up as we hear back from exhibitors. We will do our best to schedule you at your preferred time, but we ask that you be flexible if that time is no longer available. Please check your email & respond to confirm the appointment date and time! Appointment times will fill up quickly. Pre-registration fees are NON-REFUNDABLE.
