PLEASE NOTE: You need to complete and print out the appropriate OFA Exam Form to bring with you to the clinic. There is no capability to photocopy forms at the clinic. You need the following for each dog to be examined. Vet information to complete the form will be sent to you when your appointment is confirmed. Your dog’s AKC registration number. Your dog’s Sire and Dam’s AKC registration number. Your dog’s Microchip number
